Health Waiver - Informed Consent

By joining this website, and in consideration of participating in the activities and programmes of Julie Rhoades you do hereby waive, release and therefore discharge Julie Rhoades from any and all responsibility or liability for injuries or damage resulting from your participation in any activities using a face to face, live stream or download method for delivery.

You understand and you are aware that strength, flexibility and aerobic exercise including the use of equipment where specified, are potentially hazardous activities. You acknowledge being informed of the possible strenuous nature of the programme and the potential for unusual, but possible physiological results, including, but not limited to abnormal blood pressure, fainting, heart attack, injury or even death, and that you are voluntarily participating in these activities and use of equipment where specified with the knowledge of the dangers involved.

You hereby agree to expressly assume and accept all and any risks of injury and death. You are aware that you have the right to request advice from Julie Rhoades at any time, in relation to the activities and exercise being undertaken and, but not exclusively, their suitability for you, with particular regard to your health and clothing. If you choose not to take the advice, or to disregard any advice so given, you do so voluntarily and accept liability for all resulting injuries, loss or damage.

You do hereby declare yourself to be physically sound and suffering from no impairment, disease or infirmity or other illness that would prevent your participation in live streamed, on demand or face to face fitness classes or activities.

You acknowledge that you have either had a physical examination and have been given your doctors permission to participate and that you have sought medication advice, or that you have decided to participate in activity and use of equipment where specified without the approval of your doctor and do hereby assume all responsibility for your participation and activities.

You acknowledge you should consult with a physician before beginning any exercise, fitness, diet or nutrition routine, especially if pregnant, have recently given birth or have pre-existing health conditions, and that nothing contained in this website or face to face classes should be considered as medical advice or diagnosis.

You assume all responsibility for ensuring your environment, clothing and footwear is safe and suitable for participating in any activities by Julie Rhoades using a live stream, online method or face to face method for delivery and take full responsibility for monitoring your own physical condition throughout any and all activities and exercises.

 

You understand and acknowledge have voluntarily chosen to participate in a programme of progressive physical exercise, and that in doing so accept the risks identified above.

  

You should not take part in classes if:

  • You feel unwell, have recently been ill, are pregnant or recently given birth.

  • You are experiencing any pain or have any bone, joint or heart conditions that could be aggravated or worsened by physical activity.

  • You have undergone any abdominal surgery or recent medical procedures.

Booking Terms

BOOKING TERMS

1.  Timetabled classes may be booked up to 5 minutes in advance of the class start time. A minimum number of 8 is required for a class to run.

2.  Julie Rhoades will provide the agreed service(s) on the specified date(s) and time(s) requested on the online booking.

3.  All customers MUST complete a compulsory Pre-Activity Assessment Questionnaire form (ParQ) prior to attending their first class. This is accessible from the booking page and is also sent via your booking confirmation email. Should this not be completed, you will be unable to participate in the class and a session credit will not be given.

4.  Customers need to be in the venue ready to start no later than 5 minutes prior to the scheduled start. For your own safety and well-being, full completion in all warmup activities is mandatory and therefore to avoid an increased risk of injury late arrivals are not permitted entry and treated as late cancellations.

5.  All classes and courses are for ages 18+ only. Spectators are not permitted.

6.  It is the responsibility of the customer to inform the teacher of any injuries or health issues before each class.

7.  Julie Rhoades will not be responsible for lost, stolen or damaged personal property during the period of the booking or while at the venue.

8.   Do not attend classes or events if you have been unwell in the last 48 hours.

9.   Mobile phones should be put away and on silent - this time is for everyone to have free of distractions :-)

10.  By booking a class you agree that you have read and agree to the terms of the Health Waiver.

11.  We want everyone to enjoy and feel welcome in classes and strive to create a supportive environment and atmosphere. Please follow dance floor etiquette guidelines and be respectful to instructors, fellow class members and venue staff at all times.

Cancellations & Refunds

CANCELLATIONS 

1. It is the responsibility of the customer to cancel or reschedule classes by using the MY BOOKINGS page. Classes cannot be cancelled via text or other messages.

2. Classes may only be rescheduled or cancelled for a credit onto the  originating plan up to 24 hours prior to the stated class time. Credits are not given after this period.

3. Class bookings may only be "rescheduled" for a class of the same type. 

4. If a class or booked session is cancelled by Julie Rhoades Dance & Fitness, you will be notified via an email to your accounts email address and a credit applied to your originating plan that may be used to book an alternative class.

7. Cancellations are also posted on my WhatsApp channel, a link to join can be found in any class booking confirmation e-mail and the CONTACT ME page.

 

REFUNDS

1. Refunds are provided via a session credit back onto the originating plan at point of class cancellation. Card refunds are not issued.
